Almost none of the NPCs have a real impact on the game per se, it is the fandom who make theories and backstories for them, as they mostly exist for us to explore and have some minor interactions with them. This one in particular is called Monoko by the fandom, and she triggers a situation called "event" that reveals this form of her. It could be considered some sort of "shock image" at best.
